Wearable system for the gait assessment in stroke patients

AB  - The assessment of the motor performance is an essential component of rehabilitation of humans with a disability. The wearable system is a preferable solution compared to the laboratory-based instrumentation for the clinical use. We integrated a system which combines array electrodes over the muscles of interest connected to the 24-channel digital amplifier and the shoe insole with five robust pressure transducers and one inertial measurement unit per leg. The system sends signals wirelessly to a computer. The graphical interface is simple for usage and simple to interpret. The EMG patterns (muscle activities) and gait events are synchronized by the software. The EMG patterns are determined by cubic spline interpolation of data from 24-pads over the muscle. The gait events are estimated by fuzzy logics applied on the thresholds from signals coming from the insole. The system was tested and accepted by the therapist as a useful and easy clinical tool.

AB  - Objective: To determine if conventional rehabilitation program combined with functional electrical stimulation (FES) applied to the ankle dorsiflexor muscles during walking through a foot drop stimulator (FDS) would influence activation of tibialis anterior (TA) and improve walking ability in individuals with stroke-related drop foot. Design: Case series Setting: Post-acute rehabilitation hospital.  Participants: Individuals with stroke-related drop foot. Interventions: All study subjects received the conventional stroke rehabilitation program 5 days a week, 1h a day combined with walking while FES system based on multi-pad electrode applied, for 30min, during 4 weeks. The following outcome measurements were taken at baseline and at post-treatment.  Main Outcome Measures: Surface electromyography (sEMG), gait speed using 10 Meter Walk Test (10MWT), Fugl-Meyer assessment (FMA) and Berg Balance Scale (BBS).  Results: The analyzed recorded EMG signal showed an improvement in both amplitude and frequency spectrum, which indicates an improvement in muscle power of TA treated with FES therapy. Mean increase in 10MWT was 38.7% (P <.05). The initial FMA score increased from 22.9±2.4 to 28.8±3.5 (P < .01) and BBS score increased from 36.6±7.4 to 42.0±8.5 (P < .05).  Conclusions: Our data confirm that FES therapy using multi-pad electrode combined with conventional rehabilitation significantly increase muscle recruitment and improve recovery of functional locomotion in patients with stroke-related drop foot.  Level of Evidence: Level IV

AB  - Transcutaneous activation of muscles with electrical stimulation has limited selectivity in recruiting paralyzed muscles in stroke patients. However, the selectivity could be increased by the application of smaller electrodes and their appropriate positioning on the skin. We developed a method for selecting the appropriate positions of the stimulating electrodes based on electromyography (EMG). The EMG activity maps were estimated from signals recorded with two electrode arrays and two 24-channel wearable amplifiers positioned on the nonparetic and paretic forearms. The areas where the difference between the EMG maps obtained from the nonparetic and paretic arms was significant were identified as the stimulation sites. The stimulation was applied through array electrodes with magnetic holders and two wearable stimulators with four output channels each. The measures of functionality included joint angles measured with goniometers (hand opening) and grasp force measured with a multi-contact dynamometer (grasping). The stimulation protocol comprised co-activation of flexors and extensors to stabilize the wrist joint and prevent pronation/supination.

Biphasic calcium phosphate/poly-(DL-lactide-co-glycolide) biocomposite as filler and blocks for reparation of bone tissue

AB  - Composite biomaterials, like calciumphosphate/bioresorbable polymer, offer excellent potential for reconstruction and reparation of bone tissue defects induced by different sources. In this paper synthesis of calciumphosphate/poly-DL-lactide-co-glycolide (BCP/DLPLG) composite biomaterial formed as filler and blocks was studied. BCP/DLPLG composite biomaterial was produced in the form of spherical granules of BCP covered by a DLPLG layer, average diameter of 150-250 mu m. By cold and hot pressing of granules at up to 10000 kg/cm(2), blocks with fine distribution of phases and porosity up to 3% were obtained. Characterization was performed by wide-angle X-ray structural analysis (WAXS), differential scanning calorimetry (DSC), scanning electron microscopy (SEM), infrared spectroscopy (IR), and mechanical properties by defining the compressive strength. In vitro citotoxicity research was carried out on cellular cultures of fibroblasts of human (MRC5) and mouse (L929). In vivo research was performed in two steps. Reparatory ability of BCP/DLPLG in mice was examined in the first step, and then bone tissue reconstruction possibilities on 10 patients in the next step. In vitro tests showed very good fibroblast adhesion and non-citotoxicity of the composite. A material is considered non-cytotoxic if the cell survival is above 50%, and in our case it was 90%. In vivo research on mice indicated high level of reparatory ability of this composite with formation of new bone and vascular tissue six weeks after reparation. Application of this composite for healing infrabone defects of patients showed a high level of osseous regeneration.

Preparation and properties of polymeric and composite bioresorbable barrier membranes

AB  - Bioresorbable barrier membranes have been widely used in dentistry and maxillofacial surgery. When the membranes are placed over bony defects and are closely adapted by the surrounding bone face, an environment that prevents invasion of competing non-osteogenetic cells from the overlying soft tissues can be created. The aim of the present study was to establish whether the production procedure of different polymeric and composite membranes affects their physicochemical properties. The physicochemical properties were examined by differential scanning calorimetry (DSC), scanning electron microscopy (SEM) with energy dispersive spectroscopy (EDS), inverse gas chromatography (lGC) and tensile strength test (TST). The surface microstructure, dependent on the processing parameters, was perforated with smaller or larger pores. The dispersive component of the free surface energy decreased by adding bicalcium phosphate (BCP) to the polymer and, consequently, non-polar interactions with the surroundings became preferential.
